D6085 Interim Implant Crown: Billing Guidelines, Insurance Coverage, and Common Denials

Updated 12/2025

What Is Dental Code D6085?

D6085 (provisional implant crown) is applicable when an interim crown is fabricated to be worn during a period of healing prior to the final impression for fabrication and placement of the permanent implant-supported or abutment-supported crown. An interim abutment (D6051) may be associated with the interim implant crown (D6085) – typically in an anterior area.

When D6085 Should NOT Be Reported

Do not report D6085 as a temporary placed in association with the routine fabrication of an implant-supported or abutment-supported crown. However, interim implant crowns (D6085) may be used while the implant continues to osseointegrate, healing around the abutment, or when the prognosis of the implant body is in question. D6085 does not specify the material or fabrication method. It may be made chairside, laboratory fabricated, prefabricated, milled, or 3D printed.

Benefits for D6085 are frequently denied. If the interim implant crown is reimbursed, the payment amount may be deducted from the reimbursement of the definitive implant crown, or may prevent reimbursement of the definitive due to frequency limitations. Ideally, interim implant crowns would be in place less than 12 months to avoid being considered a definitive restoration under some plan language. Benefits vary greatly between plans.
